Your Claim Reference ID is printed on the letter we sent you.

What the Reference ID looks like

Your Claim Reference ID follows this format:

It always starts with ECG- followed by letters and/or numbers. Examples:

• ECG-8832
• ECG-A1B2
• ECG-12345

Where to look

The Reference ID is typically located:

• In the upper right portion of the letter
• Below the date line
• Labeled as "Claim Reference ID" or "Reference ID"

Can't find it?

If you received a letter but cannot locate your Reference ID:

• Check both pages if your letter has multiple pages
• Look for any code starting with "ECG-"

Used the QR code instead?

If your letter includes a QR code and you scanned it, you were taken directly to your offer page—no Reference ID entry needed. The QR code contains your unique link.
