Electronic Records and Signature Disclosure

Notice: This disclosure is provided in compliance with the Electronic Signatures in Global and National Commerce Act (E-SIGN Act), 15 U.S.C. 7001 et seq., and the Uniform Electronic Transactions Act (UETA).
1. Electronic Records and Signatures
By consenting to do business electronically with Eterna Claims Group, you agree to receive and sign documents electronically. This means:
- You will receive documents in electronic format rather than paper format
- You will sign documents using electronic signatures rather than handwritten signatures
- Electronic records and signatures have the same legal effect as paper documents and handwritten signatures
2. Documents Covered
The following documents may be provided to you electronically and/or require your electronic signature:
- Claim Purchase Offers
- Assignment of Claim Agreements
- Disclosure Statements
- Terms of Service and Privacy Policy
- Tax-related documents (Form W-9, Form 1099)
- Notices and Communications
- Transaction Confirmations
- Any amendments or modifications to the above
3. Hardware and Software Requirements
To receive electronic records and execute electronic signatures, you will need:
- Computer or Mobile Device: A desktop, laptop, tablet, or smartphone with internet access
- Operating System: Windows 10 or later, macOS 10.14 or later, iOS 13 or later, Android 8.0 or later
- Web Browser: Current version of Chrome, Firefox, Safari, or Edge with JavaScript and cookies enabled
- Email: A valid email address capable of receiving messages and attachments
- PDF Reader: Adobe Acrobat Reader or equivalent software to view and print PDF documents
- Storage: Sufficient storage space to download and save electronic documents
4. How to Obtain Paper Copies
You have the right to receive paper copies of any electronic documents. To request a paper copy:

Paper copies will be provided at no charge. Please allow 5-7 business days for delivery.
5. How to Withdraw Consent
You may withdraw your consent to receive electronic records at any time by:
- Emailing us at contact@eternaclaimsgroup.com
- Calling us at (312) 450-0502
- Writing to us at our address above
Important: If you withdraw consent before completing your claim transaction, we may be unable to process your transaction electronically. You may need to complete the transaction using paper documents, which may result in delays.
6. Updating Your Contact Information
You agree to keep your email address and other contact information current. If your email address changes, please notify us immediately to ensure you continue to receive important documents and communications.
7. Electronic Signature Process
Our electronic signature process is provided by Dropbox Sign (formerly HelloSign). When signing documents electronically:
- You will receive an email notification when a document is ready for signature
- You will review the document in your web browser
- You will click to place your electronic signature on designated signature fields
- Your electronic signature will be legally binding
- You will receive a signed copy of the document via email
8. Legal Effect of Electronic Signatures
By using our electronic signature service, you acknowledge and agree that:
- Your electronic signature is legally equivalent to your handwritten signature
- Documents signed electronically are legally binding and enforceable
- You intend to sign, execute, and accept the documents presented to you electronically
- You are acting voluntarily and have the legal capacity to enter into agreements
9. Record Retention
We recommend that you download and save or print copies of all electronic documents for your records. While we maintain records of your transactions, having your own copies ensures you have access to important documents.
10. Your Consent
By checking the electronic records consent box on our portal, you confirm that:
- You have read and understand this disclosure
- You consent to receive electronic records and use electronic signatures
- You have access to the required hardware and software
- You can access and retain electronic documents
- You understand how to withdraw consent and request paper copies
